Direct Primary Care (DPC) is a healthcare model where you pay a small, flat fee (starting at $80/month) directly to your Health Care Provider.

Direct Primary Care Services

Annual, personalized, in-depth, wellness examination and evaluation
20 additional acute and non-acute visits in-person or via telehealth (up to 5 per quarter)
Visits in addition to the above included visits available at the discounted rate of $10 per visit
Sick visits for DPC member’s dependent children.

Non- Medical Personalized Services. The Practice shall also provide Members with the following non-medical services and amenities:

After-Hours Access. Members will have direct telephone access to their provider for emergent concerns which unexpectedly arise after oﬃce hours. (Medical emergencies should always be directed to 911 and the Emergency Room but then don't hesitant to reach out).
Direct Electronic Access.
On-Time Appointments.
Same day/next day Appointments.
Specialist & Care Coordination.
Telehealth and In-Office Visits

For employer groups, coordinated site visits for employer wellness visits.

In Home Visit can be coordinated at the request of the patient. If determined necessary by the provider, for an additional fee.

Fees:

Enrollment: $150 per Member

- One-time, nonrefundable, includes review of patient’s medical record, and initial visit.

Ongoing:

- Adult Membership (18+) $80 per Member per month

- Child Membership (4+) $50 per Member per month with a qualifying adult membership
