​Direct Primary Care

Direct Primary Care (DPC) is a healthcare model where you pay a small, flat fee ($80/month) directly to your Health Care Provider.

 

With this you get:

  • DIRECT access to your provider (text, email, phone, portal).

  • Mail order pharmacy with MANY common generic medications included for free.

  • Functional medicine approach.

  • NO waiting room.

  • Same or Next day appointments.

  • In-office blood draws.

  • Tele-Health Visits as needed, or just text if its a quick question!

  • Personalized care because your provider KNOWS YOU!

  • Unhurried visits giving you the time to talk about all your healthcare needs.

Direct Primary Care Services

  • Annual, personalized, in-depth, wellness examination and evaluation

  • 20 additional acute and non-acute visits in-person or via telehealth (up to 5 per quarter)

  • Visits in addition to the above included visits available at the discounted rate of $10 per visit

  • Sick visits for DPC member’s dependent children. 

  • Sports Injuries and physicals

  • Prevention Care

  • Electrocardiogram (EKG)

  • Blood Pressure Monitoring

  • Chronic disease management (e.g. diabetes, high blood pressure, obesity) 

  • Urinalysis

  • Rapid Strep Test

  • Rapid Mono Test

  • Rapid Flu A/B Test

  • Rapid COVID-19 Test

  • Pregnancy Tests

  • Rapid Glucose Test

  • Rapid HgA1C

  • Rapid Lipid Test

  • Removal of Cerumen (ear wax)

  • Wound Repair and Sutures (small fee might be added)

  • Abscess Incision and Drainage (small fee might be added)

  • Basic Vision

  • Drawing basic labs

  • Discounts on supplements, IV vitamin therapies and aesthetics.

 

Non- Medical Personalized Services. The Practice shall also provide Members with the following non-medical services and amenities:

  • After-Hours Access. Subject to the limitations of the Agreement. Members will have direct telephone access to their provider for emergent concerns which unexpectedly arise after office hours. (Medical emergencies should always be directed to 911 and the Emergency Room but then don't hesitant to reach out).

  • Direct Electronic Access.

  • On-Time Appointments. Subject to the limitations the Agreement.

  • Same day/next day Appointments. Subject to the limitations of the Agreement.

  • Specialist Coordination.

 

- In Home Visit can be coordinated at the request of the patient. If determined necessary by the provider, for an additional fee.

Enrollment Fees

Per Member $150 per Member

One-time, nonrefundable, includes review of patient’s medical record, and initial visit/consultation.

Individual Membership $80 per Member

Couples $150 per Couple

Adult and child - $180

Family - $275
